Here's my stupid story.

I recently redesigned my site - yeah, yeah from a template, but I'm not a designer, what can I say.

Anyhow, when I created my order form, I forgot to add a field for the hosting package!

What a dork. Luckily I noticed it before I got any orders!

Hahaha. Ive done similar stuff like that in the past... forgetting to add important form fields.
