You generally only need the "index.php" file for the status.

Most people put the file into the /usr/local/apache/htdocs/status/

You can put the status file anywhere as long as it's readable via the browser.

You need to use the code/file that is included with your WHMCS version. The file information has changed over the years depending on your version. If you look in the /status folder of your WHMCS distribution you'll see the files that you need to upload. I believe that you can also download the status.zip file from WHMCS member area.

Posting code on a website can become outdated (especially when I know there are other version of WHMCS that use different versions), so if you use the one in your installation archive (or download your copy again) you'll have the files you need to put online.

The older versions of WHMCS used status.php, the newer versions only have a status folder. You just upload that folder to the server and then you can execute the index.php file. You can also rename the index.php to status.php, it's the same file.
