Need share host w/ SSH access

I need a share host w/ ssh access, so I can view my access/errorlog live for debugging purpose.

My requirements are:

- 2G disk/20G Bandwidth
- Unlimited or a minimum of 5 hosting domains
- 5 mysql DBs
- Mysql5/PHP5/PHPmyadmin
- 99.9% uptime

Please provide the following info:

- Your uptime stat from a 3rd party
- Where is your DC located
- Webhost or reseller
- Server spec/average load
- Is mysql DB hosted on the same server or a diff. server.
- Your uplink provider
- How long have you been in business.

Looking to pay under $8/month. Please let me know if you have a plan that fit the above.
My current hoster has been down for the past 8 hours, even their main site is down. So much for 99.9% uptime guarantee.
