What issues do you mean? I think nobody knows about that.

Data synchronization between the web frontends, session management, MySQL clustering are the obvious ones.

Are you planning to do that yourself? I think that it worth to ask web hosting company to help with that.

If you have no experience in load balancing before, its better to go with some system admin in your hosting provider if you want to avoid problems in Mysql clustering and few other areas.
